The number of patients being supported by a Care Plan following MDT intervention continues to rise, including increases in the number of Care Home patients with a plan.
Frequent Flyer data has been provided to all practices which will enable a focussed audit of patients with the highest levels of system activity, ensuring that those most vulnerable are put forward for further interventions.
The impact of the new SLA on improving MDT working will be evaluated from December in preparation for contract renewal in March 18. This evaluation will take the form of an initial ‘Survey Monkey’ to inform a Practice Manager workshop to be held at TITO in December.
