About the service
Following the successful delivery of the NHS England ‘Clinical Pharmacists in General Practice’ pilot, the GP Alliance have been sub contracted to support the newly formed Primary Care Networks in delivering the Pharmacy services over the next 5 years.
This has involved working with all pharmacy providers across the Sunderland system to establish a citywide support model and extensive recruitment to provide an integrated approach to medicines optimisation and clinical pharmacy.
Pharmacists are embedded within Primary Care Networks, resolving day to day medicine issues and delivering direct patient care. They help to manage long term conditions and provide advice to patients on multiple medications.
